Dealing with spam on phpBB

How do you deal with Spam on phpBB?

Used to use activation emails till they beat that as well.

Captcha used to work till the spammers defeated it.
Are you using custom image verfication or another system?

It is mostly spam for prescription drugs. It used to be a big thing for spamming the memberlist with links to their sites.

I haven't had a problem so far. Don't you have moderators who delete spam and ban users?
